How We May Use and Disclose Your PHI

Treatment: To dispense your prescriptions, communicate with your prescriber, and coordinate your care.

Payment: To process billing, authorize insurance, and collect payment for services.

Healthcare Operations: For quality improvement, staff training, and accreditation activities.

Required by Law: When required by federal or state law, court orders, or law enforcement.

Public Health: To report adverse drug events to the FDA MedWatch system.

Your Rights Regarding Your PHI

  • Right to Access: Request a copy of your prescription records and health information.
  • Right to Amend: Request correction of inaccurate information in your records.
  • Right to Restrict: Request we limit how we use or share your PHI in certain situations.
  • Right to an Accounting: Receive a list of disclosures we've made (excluding treatment/payment/operations).
  • Right to a Paper Copy: Receive a paper copy of this notice upon request.
  • Right to Complain: File a complaint with us or with the U.S. Department of Health and Human Services (HHS) without fear of retaliation.
